Procedure for forwarding Claims Process:

• Death: guarantee in case of Death of the insured; important death certificate.
• MA:  Accidental Death; important death certificate.
• IPDF: Functional Disability Disease; important day of total and permanent disability.
• IPA: Partial or Total Permanent Disability by Accident; important date of discharge.
• IR: Residential Fire / RCF: Family Liability; important to inform the date and place of the accident.
• Family Funeral Service: (covers funeral costs up to R$ 3,500.00/R$ 5.000,00) funeral service by requesting Customer 24h Service, phone 0800.055.2039.
